Coordinating your Care

Case Managers are a team of registered nurses who monitor your care or that of a loved one who has been admitted to the hospital. This is done throughout the inpatient stay, and we work with our patients and their families to develop a discharge plan of care tailored to their individual needs.

Our goal is to ensure a smooth and timely transition from the hospital to the home or, if necessary, to another facility.

Case managers will help arrange appropriate community and home care services such as:

  • Acute and sub-acute rehabilitation
  • Nursing home care
  • Home health nursing and physical therapy
  • Home IV infusion therapy
  • Durable medical equipment (walkers, canes, etc..)
  • Home oxygen
  • Medical alert systems
  • Questions regarding your Medicare rights and appeal processes.
  • Coordinating outpatient dialysis
  • Referrals to community providers to assist with supportive services such as transportation, protective services, home health aids, and other programs.

When It’s Time to Go Home

Some patients may need additional treatments or services after they are discharged. Nurse case managers will work with patients and their families to coordinate services needed upon discharge from the hospital.

Day of Discharge

Your physician will decide when you are ready to leave the hospital. To prepare for discharge, you should make arrangements in advance with a family member or friend to help you when it is time to go home. Here are some important things to remember:

Discharge Time – 1 p.m.: As an organization, Atlantic General has set a new goal to discharge patients by 1 p.m. each day to improve access to care for patients from the ER and OR who are awaiting inpatient care. The day you are scheduled to be discharged, to either go home or enter rehabilitation, please help us meet this goal by making arrangements with your loved ones to be present in the hospital on the morning of discharge. Your cooperation is very important to this initiative.

Please make arrangements with your loved ones to be prepared to pick you up at the time of your discharge so our staff can clean the room and make it ready for the next patient. Thank you.

Personal Belongings: Collect all of your belongings and/or medications. If you have items stored in the safe, please remind your nurse.
