Exam Image Availability

Atlantic General Hospital’s state-of-the art technology offers immediate access to the images from your exam. No longer does your physician have to wait to have copies of your films made. Although we can still provide a copy of your examination for you with 48 hours notice, your physician can now review your exam images immediately via the Internet. We offer safe and secure web access for your physician through the hospital’s network. Your privacy is assured.

The Eunice Q. Sorin Women’s Diagnostic Center is happy to provide you with images from your exams for follow-up care, insurance purposes or your own use.

Images on CD

Since imaging has moved into the digital age, it is no longer standard of practice to print films. All digital images are stored on computers. We'll put your digital images, including a viewer, on a compact disk (CD). You or your physician can view your images on any computer that has a CD-ROM drive. All image disks contain your results, if available, unless you request that they not be included.

Release Authorization Form

Health Insurance Portability and Accountability Act (HIPAA) regulations require that you complete a release authorization form before we can provide your images and information to other health care providers.

You can print the form from our web site. On the release form please indicate clearly the name and date of birth of the patient, and where the exams need to be sent. Please mail, bring or fax your signed, competed release authorization form to us. Please also provide a contact number, so we may contact you in case there are any questions.

We'll Contact You

After we receive your request or authorization form, we will contact you by phone to confirm your request. Please allow at least 48 hours for us to have your CD or films ready. If you need further assistance please call us at (410) 641-9215.

A photo ID will be required when picking up any exams. We will be happy to release records to a patient's family member as long as the patient sends a signed document, giving a family member consent to do so.

Any Costs?

Currently, the first set of films is provided at no cost to the patient. There is no charge for CD-ROMs and, of course, Internet access for your physician is always free.

If you are a patient and would like to obtain your exams from the Eunice Q. Sorin Women’s Diagnostic Center, please print and complete the Imaging Records Release Form.

If you are a patient and need to request your films from a prior facility to be sent to Atlantic General, please use the Medical Records Request Form.
