bill. Future statements may reflect charges not yet posted when your most
recent statement was generated. You have the right to request an itemized
statement of your account within one year of receipt of this summary.
Please notify the Patient Accounting Department by letter, fax or phone.
Your inquiry should contain the following information: patient name, account/reference
number, date of service and description of the item(s) in question.
During the care you received at our facility, you may incur charges from
other providers. Should you have questions concerning their bill(s), please
contact them directly.
