Preparing for your Visit

Pre-Certification
Certain services may require pre-approval from your health insurance company before they can be performed. Before your visit to the hospital, review your health plan’s booklet or call your insurance company to clarify your policy benefits and requirements. It is important for you to be aware of your plan’s pre-certification or authorization requirements in order to receive the full benefits allotted for services performed.

Co-Pays and Deductibles
If you have a co-pay or a deductible in your health insurance plan, it will be due at the time of service. Call your insurer with any questions. View this helpful video about co-pays and deductables.

To schedule your appointment in advance
Please call our Scheduling department at (410) 641-9714.

Having the following information available when you call to schedule your appointment will make the process quicker and more convenient for you:

  • Full Patient Name
  • Patient’s Social Security Number
  • Day, Evening, and Emergency telephone numbers for patient
  • Patient Address
  • Your Name, Address, and Phone
  • Name, Address, and Phone for Primary Care Physician, if different than referring physician
  • Complete Insurance Information for patient
  • Allergy Information on patient, if patient has allergies
  • Service to be Provided
  • Employer
  • Signed order from ordering physician including the diagnosis and procedure

To Prepare for your Arrival to the Hospital
Please bring with you the following items:

  • Adults (18 years or older) and Emancipated Minors: Insurance cards, driver’s license, and/or Medicare card
  • Minors: Form of identification; also, a Consent-for-Treatment will be required from the parent or legal guardian. This can be obtained in person or verbally if parent or legal guardian is not present.
  • Signed order from the physician including the diagnosis or reason for visit
  • Referrals and/or authorizations that are required by your insurance company
  • Phone numbers
  • Local or temporary address
  • Fax number for referring physician so that your results can be faxed
  • Information on Advance Directives for patient (not required for emergency or outpatient visits)
  • Names, addresses, and phone numbers of family members or friends to contact in an emergency
  • Accident information (may be needed for workers compensation or automobile claims)

Ask any questions you may have. Make sure you have a good understanding of what is covered by your plan and what you will have to pay personally.
