The Diabetes Outpatient Education Program offers a comprehensive program of group and individual educational sessions for persons with diabetes and their families. Our program has just recently been awarded the prestigious American Diabetes Association (ADA) Education Recognition Certificate for its diabetes self-management education program.

Atlantic General Hospital’s Diabetes Outpatient Education Program provides:

• Two Certified Diabetes Educators (A nurse practitioner and registered dietitian)
• A personalized assessment
• Goal setting and a plan of action individualized for each patient
• In-depth nutrition counseling and meal planning assistance
• Information regarding diabetes medications
• The how’s and why’s of checking blood glucose levels
• The tools to create a physical activity/exercise plan
• Guidance on how to cope with diabetes
• Individual and group education and counseling
• Regular health status reports to the referring provider
