factors, such as your insurance plan set up, how much of your deductible
has been met for the current plan year and the complexity of your condition.

expect on your bill.

Price transparency also helps our patients make the best decisions about
their health care. A member of our Patient Financial Services Team would
be happy to provide a price estimate. You can call us 9 a.m. to 4 p.m.,

A few tips:
- It is helpful to know the CPT code of your procedure. Your provider can
give this to you. If you have an order from your physician, it will be
included on this document as well. If you have already scheduled your
procedure with us, our staff can locate this code.
- Please allow up to two business days for your estimate. It can take extra
time to obtain all the information we need to provide you with the most
accurate estimate possible.
Do you have insurance? Start there.
Your insurance company can also help you obtain an estimate of what you
would pay for a certain procedure. Your insurance provider can determine
out-of-pocket costs since it knows the specifics of your coverage plan,
deductibles and co-pays.
